HISTORY: restless, crying, obese, hard distended abdomen, AP / ALT mildly elevated, ?senile behavious or pain?

CHARGES: abdominal ultrasound

Abdominal U/S

SUMMARY:

The findings here are highly suggestive of pituitary dependent hyperadrenocorticism with bilateral adrenal hyperplasia and hepatomegaly. Adrenal function testing is warranted here. This would account for the abdominal distension however the significance in relation to the behavioural abnormalities is uncertain. Significant dental disease is noted whilst under sedation which could be relevent in terms of potential discomfort?

LIVER: Abnormal: hepatomegaly, moderate to marked, rounded margins, coarse diffuse hypoechogenicity, occasional tiny cyst

BILIARY TRACT: Normal

SPLEEN: Normal

LEFT KIDNEY: Normal

RIGHT KIDNEY: Normal

LEFT ADRENAL: Abnormal: 9.5mm, enlarged, normal shape / margins, very prominent hypoechogenicity – almost anechoic

RIGHT ADRENAL: Abnormal 8mm otherwise as above

URINARY BLADDER: Normal

ILIOSACRAL LYMPH NODES: Normal

VISCERAL LYMPH NODES: Normal

STOMACH: Normal tiny cyst in mesentry just caudal to stomach ?significance?

SMALL INTESTINES: Normal

LARGE INTESTINES: Normal

PANCREAS: Normal

GENITAL TRACT: prostate small and hypoechoic
