The renal chanegs indicate chronic renal pathology, although azotaemia is not yet apparant; what is the urine sg? monitoring of renal function is recommended, as early stages of renal failure may explain the symptoms.  Additionally a normal pancreatic appearance would not exclude chronic pancreatitis and so fPLi measurement could be considered.
